Instructions
Step 1: Verify if your Candlsense Warmer is affected by this recall.
Step 2: Fill out and submit the Recall Form.
Step 3: Wait for Provo Craft to send you a prepaid envelope. The envelope should arrive within 1 to 2 weeks.
Step 4: Unplug, then cut off the Candlsense Warmer polarized plug.
Step 5: Return the polarized plug, and a copy of the Recall Form through the U.S. Postal Service in the prepaid envelope.

Polarized Plug
Hazard The internal heating element of the candle warmer can detach and melt the bottom of its plastic casing, which can in turn ignite or scorch the surface on which the candle warmer is placed, posing a potential fire hazard to consumers. Consumers should discontinue use of the affected candle warmers immediately.

Description The Candlsense™ Candle Warmers are small, round, black metal plates that are encased in a white plastic covering, and have a cord and a polarized plug. The candle warmers measure about 4 3/4 inches in diameter. The words Provo Craft & Novelty Candle Warmer Air Freshener Model: WCY-8888" and the date code are embossed on the underside of the warmer. The candle warmers were sold as individual units in clear plastic packages under the name "Candlsense™ Warmer". For a complete list and images of the recall products, see recalled items above.
Action Consumers should immediately discontinue use of these products. DO NOT return Candlsense™ Warmers to the location where you bought them. Follow the procedure described below to contact Provo Craft and receive a voucher worth 120% of the suggested retail price of the candle warmer. The voucher can be redeemed at www.creativexpress.com. You must contact Provo Craft directly in order to receive a voucher.

Any consumer who wishes to participate in the recall should fill out the "recall form" above and submit. You should then unplug your candle warmer from any electrical outlet and cut the cord just above the polarized plug. Discard the candle warmer and cord in the trash. Place the polarized plug and your printed recall form in a safe place.

You will receive a prepaid envelope within 1-2 weeks of submitting your recall request. Place the polarized plug and your printed recall form in the envelope, seal the envelope and place the sealed envelope in the United States Mail. Once your recall form and plug have been received by Provo Craft your voucher will be processed and you will receive it within 4-6 weeks. If you have provided us with an email address, a link and code will be sent to you as soon as the recall has been processed. If you do not provide an email address, a voucher will be mailed to you at the address you provided.
Returns Do NOT return Candlsense™ Warmer to the location where you bought them. Return them to Provo Craft, via the United States Mail, for a refund voucher. Any consumer who wishes to participate in the recall should fill out the "recall form" above and submit.

Incidents Provo Craft has received 11 reports of incidents in which the heating elements detached, including two fires and nine incidents of property scorching. One consumer reported a blistered finger.
Sold at Wal-Mart, Jo-Ann Fabrics, and other retailers nationwide from August 2006 through October 2007 for approximately $5-10.
Refund The only way to qualify for a refund is to make a claim and return your Candlsense™ polarized plug to Provo Craft. Consumers must complete the recall form, or contact the Provo Craft Claims department at 1.888.306.0132 Monday through Friday 7 a.m. to 6 p.m. mountain time. Upon receipt and verification of your claim, Provo Craft will process your refund voucher. All returns to Provo Craft must be sent by U.S. mail in the prepaid envelope. Allow 4 to 6 weeks for processing from the time your claim is received and verified by Provo Craft.
